Part 27
Everything Else You Wanted to Ask

Picture the last two minutes of an appointment. The patient has a hand on the doorknob. “Doctor, one more thing.” What follows is often the most important question of the visit: about sex, or memory, or a habit they are ashamed of, or something they read at two in the morning. This part is for the doorknob questions.
Clinical scenes in this edition are explicitly labeled illustrations. They describe imagined situations used to explain medical decisions, not documented patients, diagnoses, conversations, or treatment outcomes. They should not be read as evidence that a particular intervention works. Personal recollections are recollections, not medical records.
Evidence grade (draft mapping). 22 questions; a question with mixed evidence counts under each grade.
- 388. Is sex safe if I have heart disease? For most people with stable heart disease, yes. If you can climb two flights of stairs without symptoms, your heart can manage it.GGuideline-based / consensus
- 389. I quit smoking years ago. Did the damage stay?Much of it reverses. Heart attack risk falls by about half within a year of quitting and keeps falling. Lung cancer risk drops steadily but stays above that of a never-smoker.CObservational
- 390. Is vaping safe? What about cannabis?Vaping is less harmful than cigarettes and far from harmless. Daily cannabis use is linked to more heart attacks and strokes.CObservational
- 391. Does sunscreen slow skin aging?Yes. In a randomized trial, adults who used sunscreen daily showed no detectable increase in skin aging over four and a half years, and melanoma rates were about half in long-term follow-up.Grade pending
- 392. Should I have my genes tested?Targeted testing is valuable when your family history suggests an inherited risk. Broad consumer tests are less useful and can mislead.GGuideline-based / consensus
- 393. Should I measure my telomeres? No. Telomere length varies widely between people and between measurements, and knowing it changes nothing about your care.FNot demonstrated / insufficient
- 394. Are my kidneys aging?Kidney function often declines with age, at widely varying rates, and one in seven American adults has chronic kidney disease. Most do not know it.GGuideline-based / consensus
- 395. Are over-the-counter painkillers safe to take regularly? Not the anti-inflammatory ones. Ibuprofen, naproxen, and similar drugs raise blood pressure, strain the kidneys, cause stomach bleeding, and increase heart attack risk with regular use.ARobust human trials
- 396. Can common medications harm my memory?Yes. A group called anticholinergics, which includes many over-the-counter sleep aids and allergy pills, is linked to confusion and a higher risk of dementia with long-term use.CObservationalARobust human trials
- 397. Should I cut salt? What about salt substitutes? Most people should eat less sodium and more potassium. Potassium-enriched salt substitutes reduced strokes and deaths in a large trial.Grade pending
- 398. Are artificial sweeteners safe? They are better than sugar for blood sugar and worse than water. Some, including erythritol and xylitol, have been linked to higher clotting and heart risk in early studies.BLimited or small human studies
- 399. Should I worry about red meat and processed meat?Processed meat, yes. Bacon, sausage, hot dogs, and deli meats are classified as a cause of colon cancer and are tied to heart disease. Unprocessed red meat is a smaller concern; keep it occasional.ARobust human trials
- 400. How much water do I need?Needs vary with body size, activity, heat, food, illness, and medicines. There is no universal eight-glass prescription; urine color is only a rough guide.EEstablished clinical knowledge
- 401. Are microplastics a real health threat? They may be. In 2024, researchers found tiny plastic particles inside the artery plaque of most patients studied, and those patients had far more heart attacks, strokes, and deaths over the next three years.BLimited or small human studies
- 402. Is my body mass index a good measure? It is a rough screening tool. Waist size and body composition tell you more.CObservational
- 403. Do pets and time in nature really help? The evidence is observational but consistent. Dog owners have about 24 percent lower death rates in pooled studies, probably because dogs make you walk and make you less alone.CObservational
- 404. How long before I see results? Some changes come within days, most within three months, and the ones that matter most are counted in years.Grade pending
- 405. What are the biggest myths you hear? Here are ten, each corrected in a line.JClinical judgment / author synthesis
- 406. Why am I losing my hair, and does anything work? Most hair thinning in men and women is hereditary and hormone-driven. Two treatments have good evidence, and both work better the earlier they are started.Grade pending
- 407. Is leaking urine just part of getting older? It is common and it is not something you must accept. Most cases improve with treatment.GGuideline-based / consensus
- 408. What about my skin, beyond sunscreen? Do cosmetic treatments slow aging? They change appearance, not biology. A few things have real evidence for skin health.Grade pending
- 409. Can I slow the aging of my eyes?Yes. Do not smoke, wear sunglasses, control blood pressure and blood sugar, eat leafy greens and fish, and have regular dilated eye exams.ARobust human trials
