Part 28
What Is Coming

Picture a cardiology clinic in 2006. If I had told my patients then that within twenty years we would lower cholesterol with an injection twice a year, replace heart valves through an artery in the leg, edit a gene inside the liver with one infusion, and treat obesity with a pill, they would have smiled politely. All of it has happened. I am cautious about promises. I am also old enough to have watched impossible things become routine. This part separates what is already here from what is close and what is still a hope.
Clinical scenes in this edition are explicitly labeled illustrations. They describe imagined situations used to explain medical decisions, not documented patients, diagnoses, conversations, or treatment outcomes. They should not be read as evidence that a particular intervention works. Personal recollections are recollections, not medical records.
Evidence grade (draft mapping). 13 questions; a question with mixed evidence counts under each grade.
- 410. How do I read a “breakthrough” headline?Find out what stage the research is in. Most breakthroughs you read about are years from your pharmacy, and most never arrive.JClinical judgment / author synthesis
- 411. What is new for cholesterol?A powerful cholesterol pill arrived in 2026, and one-time gene editing is in early human trials.RRegulatory statusBLimited or small human studies
- 412. What is next for Lp(a) and blood pressure? Two more Lp(a) drugs are in large trials after the first one failed, and a blood pressure injection given twice a year is in a major outcomes study.Grade pending
- 413. What is the next wave of weight-loss medicine?Pills, monthly injections, stronger combinations, and, eventually, lower prices.RRegulatory status
- 414. What is coming for Alzheimer’s disease?Earlier detection by blood test, treatment at home, and, most important, trials testing whether treating people before symptoms can prevent the disease.Grade pending
- 415. Are cancer vaccines real?Yes. Personalized vaccines built from a patient’s own tumor have now succeeded in a Phase 3 trial in melanoma.Grade pending
- 416. What is CAR-T, and why does it matter beyond cancer? It is a treatment that reprograms a patient’s own immune cells to destroy a specific target. It has cured some blood cancers, and it is now putting severe autoimmune diseases into remission.ARobust human trialsBLimited or small human studies
- 417. Will we be able to replace organs?Possibly, with organs from gene-edited pigs. The first formal clinical trials of pig kidneys are under way.BLimited or small human studies
- 418. Is anyone testing a true anti-aging drug in people?Yes, in several ways, though none has reported a result that would change practice.Grade pending
- 419. What new technology will change my checkups?Your watch will screen you for more conditions, heart scans will be read by software that measures plaque, and risk prediction will become more personal.Grade pending
- 420. Realistically, what will be different in five years and in fifteen? In five years, better versions of what works now. In fifteen, perhaps the first treatments that target aging itself.JClinical judgment / author synthesis
- 421. What should I do while I wait?Build the bridge. Be alive, strong, and clear-minded when the next advance arrives.JClinical judgment / author synthesis
- 422. How will I know when something is ready for me?When the evidence addresses the outcome and population relevant to you, regulatory status permits its use, and a clinician explains why the expected benefit outweighs the harms and burden in your situation.GGuideline-based / consensus
