Part 8
Sleep Apnea, the Silent Thief

ILLUSTRATION. Picture a man who blames his poor concentration on work. At night he snores loudly and wakes unrefreshed, but he has never connected those hours with the ones at his desk. Asking about sleep does not establish a diagnosis. It reveals a reason to investigate one.
Clinical scenes in this edition are explicitly labeled illustrations. They describe imagined situations used to explain medical decisions, not documented patients, diagnoses, conversations, or treatment outcomes. They should not be read as evidence that a particular intervention works. Personal recollections are recollections, not medical records.
Evidence grade (draft mapping). 9 questions; a question with mixed evidence counts under each grade.
- 107. What is sleep apnea?A condition in which the airway collapses again and again during sleep, cutting off breathing for ten seconds or more each time, sometimes hundreds of times a night.EEstablished clinical knowledge
- 108. How common is it?Very. Roughly 30 million American adults are estimated to have it, and most of them have never been diagnosed.CObservational
- 109. Why does a cardiologist care so much about sleep?Because untreated sleep apnea drives high blood pressure, atrial fibrillation, heart failure, stroke, and insulin resistance.CObservational
- 110. What are the signs?Loud snoring, pauses in breathing that someone else notices, waking up gasping, morning headaches, a dry mouth, getting up often to urinate at night, and sleepiness during the day.Grade pending
- 111. How is it diagnosed? Can my watch tell me?With an overnight test, which can now usually be done at home. Some watches and rings can flag possible apnea, but they do not make the diagnosis.Grade pending
- 112. Does CPAP actually help?Yes for symptoms, blood pressure, and quality of life. The evidence that it prevents heart attacks and strokes is weaker than we expected, probably because many people in the trials used it only a few hours a night.ARobust human trials
- 113. What if I cannot tolerate CPAP?There are good alternatives: a dental device, weight loss, positional therapy, a weight-loss medicine, an implanted nerve stimulator, and in some cases surgery.Grade pending
- 114. Does mouth taping work? No. There is very little evidence of benefit, and it can be harmful if your nose is blocked or you have undiagnosed sleep apnea.BLimited or small human studies
- 115. Is snoring by itself a problem? Not always, but loud nightly snoring deserves a test, because the only way to know whether you are also pausing is to measure.GGuideline-based / consensus
