Question 66 of 433
Do I need a “full” thyroid panel with reverse T3 and antibodies?

The short answer
SKIP it unless your TSH is abnormal or you have clear symptoms. TSH first is the right approach.
Reverse T3 has no validated use in ordinary practice. Thyroid antibodies can predict future underactivity but rarely change what is done while the TSH is normal. “Optimal” TSH ranges narrower than the laboratory’s are not supported by outcome evidence. I have, in the past, written about tighter targets; the evidence has not borne them out, and I no longer chase them.
